Blood Pressure Classification 2026: Interpreting Systolic, Diastolic, and Pulse Pressure

High blood pressure (hypertension) is often termed the “silent killer” because it frequently produces no noticeable symptoms while causing cumulative microvascular damage to the heart, brain, kidneys, and peripheral circulation.

Understanding Blood Pressure Numbers

Blood pressure readings consist of two distinct numbers measured in millimeters of mercury (mmHg):

  • Systolic Pressure (Top Number): The peak pressure exerted against the arterial walls when the heart contracts (ventricular systole) and pumps blood into systemic circulation.
  • Diastolic Pressure (Bottom Number): The baseline pressure inside the arteries when the heart rests between beats (ventricular diastole) and ventricles refill.

What is Mean Arterial Pressure (MAP)?

While systolic and diastolic pressures measure peak and trough pressures, Mean Arterial Pressure (MAP) represents the average perfusion pressure driving blood through tissue capillary beds throughout an entire cardiac cycle:

MAP = Diastolic Pressure + (1/3 × [Systolic Pressure – Diastolic Pressure])

A MAP of at least 65 mmHg is physiologically required to maintain adequate cerebral, myocardial, and renal tissue perfusion.

What is Pulse Pressure?

Pulse Pressure (PP) is the numerical difference between systolic and diastolic pressure (PP = Systolic – Diastolic). A consistently wide pulse pressure (greater than 60 mmHg) is a strong independent predictor of arterial stiffness and cardiovascular events, especially in older adults.
